recheck eyes and leg in one week

2024-10-28 09:32 AMS

Left leg wound has a 1-2mm scab remaining, but is otherwise healed. P did not pick at the region over the weekend, The surrounding skin still has a light yellow discoloration, but the skin is soft and pliable and there are new feathers growing in. OS there is still a large area of corneal scarring and vascularization present. FS was negative. Ok to move to aviary. Plan to recheck in 1 week, or sooner if any concerns noted.

Removed bandage. Duoderm had slipped distally a little bit. Wound looks good, nice granulation bed, a little smaller than 2 days ago. Flushed area with LRS. Removed two sutures that were no longer holding edges together. Flushed again. Did laser tx. Applied Duoderm over the wound, SSD cream in very thin layer on skin surrounding wound. Covered with telfa, then wrapped with cast padding and vetrap. Checked around neck for any issues with e-collar, none seen.

Bandage Change & Wound Assessment:

E-collar and bandage in place. Removed bandage from L leg. There is a 1cm area at the distal aspect of the wound site that is open with a nice, deep pink granulation bed present. The surrounding skin is discolored yellow, and futher from the incision is slightly greenish (hopefully resolving bruising), but appears viable and well-adhered to the underlying tissues. Cleaned the area with dilute betadine. Performed laser tx. Applied Duoderm to wound, SSD in a very thin layer to surrounding skin. Covered with telfa and bandage. Recheck in 48hr.

Bandage was clean and intact. Removed bandage material. Incision showed moderate to marked dehiscence in the center where sutures had been torn out. Previously noted inflammation at the distal end of the incision has decreased. Mild bruising is present around the middle of the incision. Per Dr. Oster, will plan to treat as an open wound. Applied triple antibiotic ointment and silver sulfadiazine to the incision directly. Applied Telfa pad, cast padding, vetwrap, and secured distally and proximally with bandage tape.

Performed exam and re-bandaging. Bandage was clean and intact externally. Once bandage material was removed, leg incision appeared clean and dry, much less inflamed than yesterday, less warm compared to prior. Sutures still intact except for the one that had been torn yesterday, and wound bed appears to be granulating in. Administered lidocaine / sodium bicarbonate topically for analgesia, then applied 1 simple interrupted suture at distal aspect of wound. Applied triple antibiotic ointment to suture line, silver sulfadiazine surrounding. Applied Telfa pad, cast padding, Vetwrap, and bandage tape at distal and proximal aspects. Prepared new, longer cone for patient.

Performed examination and bandage change. This morning, patient had pulled bandage down to his ankle, but had not unraveled it like yesterday. Removed bandage material. One suture was removed at the distal end of the wound, leaving a roughly 3x2mm gap. Cleaned wound with dilute betadine to remove debris, blood. Performed laser therapy as described above. The left leg was still somewhat warmer than the right, but not as much as yesterday. Applied triple antibiotic ointment to incision and silver sulfadiazine surrounding. Applied Telfa pad, cast padding, Vetwrap, and taped both ends of the bandage.

Performed bandage change and examination on left leg soft tissue injury closed on 10/9/24. Bandage appeared intact and clean. After bandages were removed, the wound appeared clean, dry, and mildly inflamed. The incision was not apposed as well as expected, but was adequate for healing given time since surgery. There was moderate bruising present around and proximal to the surgical site. Gently cleaned site with dilute betadine to remove blood and debris. Performed laser treatment as described above around surgery site. Applied triple antibiotic ointment to the incision, and silver sulfadiazine to the periphery of the incision. Applied Telfa pad, cast padding, Vetwrap, and silk tape bandaging. Will plan to check bandage again on 10/14/24.

Surgery report
Procedure - Wound debridement
Performed by - Sokoloff
Surgery time - 45 minutes

Pre-surgical vitals: HR 350 bpm, RR 45 brpm
Lubricated eyes, placed in dorsal recumbency. Patient had already
removed most bandaging material over the left leg wound, so removed
what remained except for sutures. Gently scrubbed the area with
dilute betadine. Irrigated with sterile saline. Removed sutures.
Gently abraded the wound bed with gauze to stimulate bleeding
and growth. Removed approx. 1mm tissue from edges of wound to
stimulate tissue growth and healing. Irrigated with saline again
to ensure cleanliness. Undermined subcutaneous tissue to allow
tension-free closure. Apposed edges and sutured in one-layer
closure using simple interrupted pattern. Applied triple-antibiotic
ointment to surgical site, Telfa pad, cast padding, and Vetwrap
with tape at distal and proximal ends of bandage. Patient recovered
quickly and with no obvious complications.

Electrophoresis Comments:
The total protein is increased. This may reflect mild patient or sample dehydration. By % TP, the alpha 2, beta, and gamma globulins are increased. This is reflective of acute inflammation and a humoral response likely to an infectious agent. Considerations inlcude acute hepatitis, aspergillosis, and chlamydophilosis. Followup EPH to monitor the A/G ratio should have prognositc value in this case.

Interpretation:
WBC has decreaed since presentation. Total protein still high consider dehydration versus acute inflammation. Dehydration less likley since PCV has normalized. Beta and gamma globulins high, consider acute inflammation. Most likely cause of inflammation parameters increased is the soft tissue wound of the left leg. Less likely due to infectious causes such as Aspergillosis and Chlamydophilas.

Procedure - Wound debridement
Performed by - BS / AS
Surgery time - 60 minutes

Prepped wound debridement site with betadine solution. Added
sterile lubricant to wound site to aid in debriding scab. Pink
granulation tissue was present underneath. Two simple interupted
sutures were placed on cranial aspect of wound. Then two horizontal
mattress sutures were placed just caudal to the interupted sutures.
4-0 PDS suture was used. Stents were made from a T-port line
to aid in tension relief of the horizontal mattress sutures.

Wound on medial aspect of the second digit was examined and Vetericyn
ointment was applied. No further treatment was warrented at this
time.

Patient recovered uneventfully from anesthesia.

The count obtained by the hemacytometer method did not appear accurate upon review of
blood smear. The slide estimate has been reported as the final result for this case.WBC clumps were observed on the blood smear. This may affect the differential and total count.
Blood parasites noted (Haemoproteus - Moderate)
Reactive monocytes (few)
Toxic change, heterophils 1+ to 2+ (few)

Electrophoresis Comments
This EPH is markedly abnormal. A low albumin is present with increases in alpha 2, beta, and gamma
globulins. This is reflective of acute inflammation and a humoral immune response likely to an
infectious agent. Considerations include acute hepatitis, aspergillosis, and chlamydophilosis.
Followup EPH to monitor the A/G ratio should have prognostic value in this case.
